What "All-on-4" is paying for

All-on-4 is a way of restoring a whole arch of teeth on four implants, two of them placed at an angle so that they anchor in the bone that remains rather than in bone that would first have to be rebuilt. That angulation is the entire point of the design, and it is also the reason the protocol keeps coming up for patients who have been told at home that there is not enough bone for implants.

When you pay for an arch, you are paying for several distinct things that happen to be sold as one package: the implants themselves and their components; the surgery to place them; the planning that precedes it; and the prosthesis — the fixed bridge of teeth that is screwed onto those implants and that you will actually live with. The prosthesis is not an accessory to the implants. In cost terms it is frequently the larger half, which is why the material it is made from changes the price of the whole case.

The published prices

Brianza Dent publishes its dental prices rather than releasing them only on request. These are per arch, with the fixed prosthetic structure included, and the two figures are the two prosthetic materials — porcelain and zirconia.

Across the dental price list as a whole, the declared range runs from 120 € to 4,120 €. Read that range carefully: the top of it is one arch of All-on-6 in zirconia, not a whole mouth. If your case involves both arches, the arithmetic starts from two.

What moves the figure in your case

A published price is a starting point, not a quotation. Five things decide where your own case lands, and none of them can be assessed from a web form.

How many implants, and in which arch

Four or six is a clinical decision taken from your CT data — bone volume, where that bone actually sits along the arch, and how the finished bridge will be loaded when you chew. Upper and lower jaws behave differently, and a case that is straightforward below can be demanding above.

Whether the bone has to be prepared first

Guided bone regeneration, a sinus lift in the upper jaw, and other reconstructive steps are separate procedures with their own cost. Part of the appeal of angled placement is that it can sometimes avoid them — but only sometimes, and only your imaging will say.

Extractions and the state of what is there now

Teeth that have to come out, roots that have to be removed, existing infection that has to be resolved: each is a line on the estimate. So is any tooth you are keeping, which has to be assessed alongside the new work rather than ignored.

The prosthetic material

Porcelain or zirconia is the difference between 2,940 € and 3,420 € on an All-on-4 arch, and between 3,640 € and 4,120 € on All-on-6. It is a genuine choice, and it is worth having the conversation about it properly rather than defaulting to the cheaper or the dearer line.

Sedation or general anaesthesia

Treatment under sedation or general anaesthesia, with an anaesthesiologist present, is a distinct item. For patients who have avoided dentistry for years, it is often what makes the treatment possible at all — but it belongs on the estimate rather than being discovered later.

The costs that are not on the dental estimate

The mistake that ruins otherwise sensible comparisons is treating the treatment price as the total. It is not. Add, honestly and in advance:

• Flights. Tirana is served by direct flights from London and other UK airports, at around three hours' flying time — but for two people, on dates you cannot freely move, at short notice.
• Accommodation and living costs for the length of each stay, for you and for anyone travelling with you.
• A second trip. Full-arch treatment is normally two visits: the surgical one, and a shorter one for the definitive prosthesis after healing. Between them there is a healing period of roughly three to six months, depending on bone quality, the surgery performed and whether grafting was needed. That interval is biological, and no clinic can compress it.
• Travel insurance that actually covers a planned medical procedure abroad. A standard holiday policy usually will not, and finding that out afterwards is expensive.
• Time off work — for both trips, not just the first.

Do the same exercise on the UK quote you are comparing with, including its own review appointments, and you will be comparing two totals rather than two headlines.

How to read a quote so it can be compared

Ask for the estimate itemised, and check that each of these appears as its own line: the number of implants and which arch; any extractions; any grafting or sinus lift; the prosthesis and its material; the temporary prosthesis, if one is part of the plan; anaesthesia; the review appointments; and the warranty terms, stating what is covered, for how long, and who pays for return travel if revision work is needed. A single number with the word "from" in front of it cannot be compared with anything.

Then ask when the figure is confirmed. Why the difference between countries exists

The honest answer is unglamorous: the cost base is different. Premises, salaries and laboratory work cost less in Tirana than in London, and the dental laboratory here is inside the building rather than a subcontractor, so the prosthetic work does not carry an external margin or a courier's turnaround. The clinic has operated under the same clinical direction since 1998, and it is accredited by QK-CSA — the National Centre for Quality, Safety and Accreditation of Health Institutions — as the only dental clinic in Albania to hold that accreditation, and separately licensed as a Day Hospital.

What the price difference is not is a shortcut through the biology. Healing takes what it takes; the number of visits is what it is.
